01 Remote Monitoring Technology

Our state-of-the-art dialysis machines are equipped with sophisticated monitoring systems that provide real-time data on clinical metrics during each treatment session. Visible to both patients and Vitality nurses, these machines track crucial parameters such as vital signs, machine pressures, alarms, and water removal, allowing for detailed, real-time monitoring of each treatment. Alerts and notifications are generated for irregularities, ensuring potential issues are addressed promptly.

02 Training For In-Home Monitoring

Patients will receive thorough training from our experienced professionals. This training includes education on how to monitor their dialysis access, their dialysis machine, recognize signs of complications, and understand the implications of their vital signs. Patients will be taught how to analyze their own metrics, report any changes, and how to get immediate help - fostering a sense of confidence and independence in managing their treatment.


03 Home Visits and Telehealth

Vitality Dialysis maintains open lines of communication with patients through regular in-home visits and telehealth encounters. Our clinical team is readily available to help assess, answer questions, address concerns, and provide guidance as needed. By blending in-home visits with telehealth technology, we can effectively evaluate the status of your treatments, make adjustments in real-time, and discuss any challenges you'd like to make to your plan of care.


04 Collaborative Care Team

Each patient is assigned a dedicated care team that includes a nephrologist, registered nurse, social worker, dietitian, quality manager, and procurement manager who collaborate to monitor and evaluate each individual’s health status and plan of care. Each of these team members will routinely connect with the patient, helping cultivate personalized care that adapts to the patient's evolving needs. The team closely reviews the data collected during treatments and assessments to analyze the care plan and make necessary adjustments to enhance the patient’s dialysis experience.


05 Emergency Response Plan

In the event of an emergency or unexpected complication, Vitality Dialysis has a comprehensive response plan in place. Patients are trained to recognize signs that require immediate medical attention and know how to contact help. Our coordinated response plan ensures that patients receive timely interventions, enhancing their safety and well-being.
