Our in-Home Care Model

We are proud to come along side patients and families and help alleviate the burdens of chronic treatment. We won't ask you to come to a clinic or office. By offering all training and treatments directly in the home, aligning with your schedule, we hope to remove the challenges of time, travel, and cost from your dialysis experience.

Embrace Independence with Home Dialysis

Vitality Dialysis is dedicated to upholding these five essential core values that we believe create an exceptional home dialysis experience:

A man is sitting on a couch in a living room.

Comfort

There is no comfort like being at home—your chair, your bed, your couch, on your schedule, and on your terms. No clinic can replicate the sense of comfort and familiarity that your home provides.

Convenience

Dialysis schedules are limited by the facility’s availability, forcing patients to adapt to inconvenient times. Short-staffing, restricted chair access, and location challenges make in-center treatment a burden for patients and their support systems.

A heart with a shield and a cross on it.

Control

When starting dialysis, patients often feel a loss of control, overwhelmed by new routines and lifestyle changes. Limited by clinic schedules, they are forced to adjust their lives to fit into assigned treatment times. Home dialysis restores control back to  the individual receiving treatment.

Choice

For most in-center dialysis patients, their freedom to make decisions is greatly reduced. The facility dictates much of the dialysis experience, impacting the patient's ability to work, travel, plan family/social events, attend other appointments, or even enjoy holidays.

Confidence

Confidence is key for patients to stay committed, remain hopeful, and achieve successful outcomes. Feeling empowered and supported helps patients remain committed to their home dialysis choice and its benefits.

01 Education

Learn About Your Options

Begin by exploring ALL your treatment options: (1) Home dialysis (hemodialysis and peritoneal dialysis) and (2) In-center dialysis. We believe that understanding your dialysis options is imperative and helps empower you to manage your health and outcomes. We encourage you to explore outsetmedical.com and kidney.org, which provides essential insights into kidney disease, dialysis, and the Tablo system.

02 Discussion

Talk to Family, Friends, and Nephrologist

It’s essential to involve your family, support system, and nephrologist when evaluating dialysis treatment options. These decisions impact your daily routine, lifestyle, and overall health, making open discussions vital. Your family and caregivers can help ensure the chosen option aligns with your lifestyle, while your nephrologist will guide you toward the most appropriate medical choice based on your unique needs.

FAQs

  • What is home dialysis?

Home dialysis is a treatment option for patients with kidney failure, allowing them to perform dialysis treatments at home instead of traveling to a clinic or hospital. It provides greater flexibility and control over your treatment schedule. Vitality Dialysis supports both self-care and staff-assist care models.

  • What home treatments options are available?

    (1) Home Hemodialysis (HHD): Uses a machine and dialyzer (artificial kidney) to filter your blood outside your body, similar to in-center hemodialysis, but performed at home. Accepted vascular access types: CVC (catheter), AV fistula, AV graft.

    **COMING SOON**

    (2) Peritoneal dialysis (PD): uses the lining of your abdomen (the peritoneum) to filter waste and excess fluids from your blood. A cleansing solution, called dialysate, is introduced into your abdomen through a catheter, and as it absorbs waste products, it’s later drained out and replaced with fresh solution. Accepted access: PD catheter only.

  • Am I a candidate for home dialysis?

There are several important factors to consider when determining if home dialysis is right for you:

    (1) Medical Condition: You need to consider your over-all stability and health to manage home dialysis. Ask your family, friends, and nephrologist to help evaluate your suitability for home hemodialysis or peritoneal dialysis based on your medical history and condition.

    (2) Self-Management at home: Home dialysis requires you and/or a care partner to be comfortable handling equipment and supplies, monitoring your own treatments, and routinely collaborating with your dialysis care team.

    (3) Lifestyle and Independence: Home dialysis offers treatment flexibility, including when and how much you dialyze. This freedom and autonomy are ideal for patients who want more control over their treatments and the liberty to continue working, traveling, or engaging in hobbies.

    (4) Home Environment: You’ll need a clean, safe space to perform dialysis and store the necessary supplies. For home hemodialysis, you’ll also need a reliable water source, electricity, and internet.

    (5) Support System: Having a strong support system, like family or friends, can help you manage home dialysis more effectively, though some patients prefer to perform treatments independently.

  • What are the benefits of home dialysis?

    Some benefits include:

    • Greater flexibility to dialyze when it's convenient for you
    • Ability to dialyze in the comfort of your own home
    • Flexible on-demand dialysis structure, allowing for additional or extended treatments as necessary
    • No traveling to dialysis clinics - alleviating time, cost, and scheduling burdens
    • Greater freedom with diet and fluid intake
    • Reduced infection risk compared to busy clinic exposure
    • Fewer hospitalizations
    • Improved blood pressure control with customized in-home treatment plan
  • How long does training take?

Training duration varies depending on the type of dialysis, and the availability of the patient and care partner to train. Most patients complete training in about 3 weeks. Your training will ensure you feel comfortable and confident in performing your home treatments.
